What the role involves

Metriport is an open-source data intelligence platform that helps healthcare organizations access and exchange patient data in real-time. We integrate with all major US healthcare IT systems and tap into comprehensive medical data for 300+ million individuals.

We've found product-market fit with multi-million ARR, 100+ customers (including Amazon One Medical, Strive Health, Circle Medical, and Brightside Health), backing from top VCs, and years of runway. We're ready to scale. We're a tight-knit, high-performing team of mostly former founders (including two YC alumni). We're engineering-heavy, operate with minimal bureaucracy and high autonomy, and hire based on competence, not prestige. We push hard—founders work six days a week from our SF office—but give everyone freedom to craft their schedule. We measure output and we're committed to sustainable intensity.

About the role

This is the first AI Engineer role at Metriport, and one of the highest-leverage hires we'll make this year. We're looking for an engineer who treats AI agents as leverage — someone who will literally change how we ship products and how we operate.

We are building a multi billion dollar company, and you would be instrumental in that. This is a huge opportunity for the right person, someone with owner mentality/high ownership, laser focused, and high energy and determination.

If we hire well, every person at Metriport effectively gets a teammate that never sleeps, we leverage new opportunities, and we discover new ways to deliver value.

What you'll do

Build the autonomous SDLC pipeline that takes tickets through to PRs — Linear as the state machine, Claude Code (or whatever's best at the time) as the execution engine, sandboxed compute, code review, CI feedback loops, the whole loop.

Collaborate with the rest of the engineering team to integrate AI to our products in ways we haven’t even conceived yet.

Build internal agents and copilots that take work off the team's plate: monitoring the platform and preparing fixes, triaging customer issues, drafting responses, generating compliance evidence for compliance audits, summarizing PR activity, on-call assistance — anything where a human is currently doing rote pattern-matching.

Design the eval, observability, and guardrail layer for everything you ship. AI systems that touch production code, customer data, or money need to be measurable, debuggable, and safe to roll back.

Work directly with founders and team leads to figure out which workflows are highest-leverage to work on next, and ruthlessly prioritize. You won't get a backlog handed to you — you'll go find the work.

About you

You're entrepreneurial-minded, with an olympian-level work ethic. You measure your output by what you actually unblock, not what you produce.

You've shipped real, production-grade systems with LLM agents at the core — not demos, not prototypes. You know what fails in production, and you've got opinions about evals, prompt versioning, tool use, retries, and cost control.

You're a strong full-stack engineer first, AI engineer second. You can stand up a service, write the IaC, instrument it, and own it end-to-end. TypeScript / Node.js, Python, AWS, and SQL aren't going to scare you.

You think in workflows and systems. You can sit in on a customer support call or a PR review and immediately spot the three steps a model could be doing.

You have a strong sense of ownership. When someone scopes a project at 3 weeks, you ask yourself "why can't it be done in 3 days?"

You care more about leverage and impact than about which model or framework is en vogue this month. You'll pick the boring tool when boring is right.

Our open-source intelligence platform helps organizations access, analyze, and exchange patient data while integrating with all major healthcare IT systems. If you’ve been to doctors in the US, you may have encountered the problem: how does a healthcare provider get access to your up-to-date medical history to treat you properly? This includes getting notified when a patient is discharged from the hospital, picks up their prescription from the pharmacy, or has a new lab result available. Through this data, care teams get insights across their patient population, helping with 30-day readmissions prediction, care gap closure, risk adjustment, and more.
